Primary Care Integration

In the Primary Care Integration model, licensed behavioral health professionals are integrated into primary-care settings to provide patient screenings and brief treatment for mental health and substance-use disorders that can be adequately addressed in those settings. Our behavioral health consultants also help coordinate referrals for those patients who need it, to appropriate specialty behavioral health services, as well for those patients transferred back to primary-care providers from specialty services for the ongoing management of physical health issues and for maintaining treatment of such mental health conditions as bipolar disorder or schizophrenia, once those conditions have been stabilized.

As part of that same integration model, each of Access East’s care-management teams includes at least one Social Work Care Manager, or else as-needed access to a Social Work Care Manager.
